If you have received a email and/or temporary password from one of the JCH Medical Group Team members you have been preregistered
. This gives you immediate access and you may begin using the Patient Portal.
All other patients must register
before proceeding. See top Left Hand corner from log in Screen select Register Tab.
Family Management allows all members of a family to be managed on one account. To add a member of your family to your Account please click the Family Management link and send JCH Medical Group the name and date of birth for each family member you wish to add to your Account.

If you are currently a patient at any of our clinics, please contact a receptionist to assist you in generating a "Patient Portal PIN." (The PIN # is not your Password)Once you have received your pin, click
and follow the instructions.
